ADHD Treatments Surrey Ltd (we/us/our), are committed to respecting your privacy and protecting your personal data. We recognise that your personal data is your property and that you have loaned it to us for specific purposes. Unless otherwise required by law, the Information Commissioner’s Office (ICO) guidance or best practice, or in order to perform our contract with you, we will only process your personal data in the way we tell you or in the way you ask us to, and we will give it back to you at any time

Details about the healthcare professional that referred you to our service
Financial information, when such as payment related information
Details about your occupation, education, interests, partners and children that are relevant to your assessment
Details about your driving history, criminal convictions and pending legal cases
Details about the first names of your family members, any family history of medical problems that is relevant to your health
Details about your past medical history, including mental health difficulties and any trauma that you may have experienced
Details about your alcohol, substances and illicit drug use
Details about any medication you are prescribed and any allergies you have
Details about any medication that I have prescribed for you and any ongoing monitoring that is required as part of the treatment
Details of any financial transactions that you have made to ADHD Treatments Surrey Ltd
How we get the personal information and why we have it:

Patient Summary from your GP to understand your past medical history
Details from any relatives or friends that are interviewed with your permission as part of the ADHD assessment

We use the information that you have given us in order to complete an ADHD assessment or provide treatment for ADHD
We may share this information with you and your GP. If you give consent we will share your information with other healthcare professionals that you would like to be involved in your care. If during an assessment, you disclose information that is pertinent to ensure the safety of you or another person, we may disclose the necessary information to the appropriate agency i.e. social services, health services, police.
Under the General Data Protection Regulation (GDPR), the lawful bases we rely on for processing this information are:
(a) Your consent. You are able to remove your consent at any time. You can do this by contacting ADHD Treatments Surrey Ltd.
(b) We have a legal obligation. We will use reasonable endeavours to notify you before we do this, unless we are legally restricted from doing so
We do not sell, rent or otherwise make personal information commercially available to any third party.
How we store your personal information
Your information is securely stored in our internal management information systems. All personal data is stored on Cloud-based servers located in the UK
Health records are retained for 30 years or 8 years after the patient has died, starting from the patient’s last appointment or their discharge from the service.

Your right of access - You have the right to ask us for copies of your personal information.
Your right to rectification - You have the right to ask us to rectify personal information you think is inaccurate. You also have the right to ask us to complete information you think is incomplete.

Your right to restriction of processing - You have the right to ask us to restrict the processing of your personal information in certain circumstances.
Your right to object to processing - You have the right to object to the processing of your personal information in certain circumstances.
Your right to data portability - You have the right to ask that we transfer the personal information you gave us to another organisation, or to you, in certain circumstances.
You are not required to pay any charge for exercising your rights. If you make a request, we have one month to respond to you.
