Assessment for ADHD 

We offer diagnostic assessments for ADHD for children over the age of 6 and adults of all ages. Our assessments are performed to a high standard and meet the recommendations set out in the National Institute for Health and Care Excellence (NICE) guideline 87 Attention deficit hyperactivity disorder: diagnosis and management, The ADHD Assessment Quality Assurance Standard for Children and Teenagers (CAAQAS) and The Adult ADHD Assessment Quality Assurance Standard (AQAS).

All child assessments are conducted face-to-face in our office in Redhill. The appointment usually takes between 2 and 3 hours. 
 
As part of the assessment, the clinician will complete a detailed social, medical and mental health history, including difficult life experiences your child may have had and a discussion about substance use if applicable. In addition, they will ask questions about family history and current living situation and complete a developmental history from early childhood. A diagnostic interview for ADHD in young people (Young DIVA-5) for 6–17-year-olds will also be completed. 
 
At the start of the assessment, your child will be asked to complete a web-based test called QbCheck with one of our colleagues. The test combines a Continuous Performance Task with motion tracking to objectively measure the three core symptoms of ADHD: activity, attention and impulsivity. The test takes 20 - 25 minutes.

Whilst your child is completing the test, you will have the opportunity to have a private conversation with the clinician to discuss your child’s strengths and difficulties at home, school, and socially. 
 
The clinician will then spend some time with your child, observing their behaviour and asking them questions about school, home life, their interests, friendships, and mood. Depending on the child’s age, their ability to engage, and what everyone is comfortable with, the clinician may ask to see the child on their own to enable them to be open and honest in their answers. 
 
For the majority of patients, the clinician will confirm the outcome of the assessment at the end of the appointment. If they are not able to do this for whatever reason, they will let you know and discuss next steps. 
 
Regardless of whether ADHD is diagnosed or not, the clinician will formulate a care plan for your child to address the needs highlighted during the appointment. These may include environmental modifications, medication (if a diagnosis is made), or signposting for relevant psychological interventions as appropriate. This will be followed up with a short report sent to you and your GP within 2 weeks and a comprehensive report within 6 weeks. 
 
If a diagnosis is made and medication is something that you would like to consider for your child, the clinician can discuss the appropriate options with you and take your child’s baseline observations. Note that is the assessment has already taken 3 hours before medication is discussed, you will be asked to book a paid follow-up appointment to discuss medication further.

*On occasion, we may not be able to offer your child an assessment if we think the support they require exceeds what we are able to offer. We will let you know if that’s the case once we have reviewed their forms. 
Adult assessments can be conducted both face-to-face in our office in Redhill and via videoconference, depending on patient preference. The process and cost for each type of appointment is exactly the same. The appointment usually takes between 2 and 3 hours. 
 
As part of the assessment, your clinician will complete a detailed social, medical and mental health history, including difficult life experiences you may have had and a discussion about substance use. In addition, we will ask questions about family history and current living situation and complete a developmental history from early childhood. A diagnostic interview for ADHD in adults (DIVA-5) will also be completed. 
 
To confirm a diagnosis of ADHD we must establish childhood onset of ADHD symptoms (between the ages of 5-12 years old) and so where possible, please bring someone to the appointment who can help to provide information about your childhood. 
If no one can attend, it is helpful if there is someone that can complete a questionnaire about your childhood development and behaviours in childhood. School reports from primary school and early secondary school are often very informative and we would ask that these are sent to us before the assessment if available. 
 
If you wish to involve your parent(s), partner, or friend, we would advise that they attend the second half of the appointment to help with the completion of the ADHD assessment. At that time, we will discuss your childhood and schooling. We will not discuss any matters explored in the first half of the assessment without your consent. They can join the appointment remotely via a video link and would only be needed for about 40 minutes. Not involving a parent, partner, or friend may, in some cases, delay the outcome of the assessment.

Starting medication 

Our clinicians initiate medication for patients with a confirmed diagnosis of ADHD in children over the age of 6 and adults of all ages. 
 
The diagnosis can be made by us, another private or NHS service, or a clinician from abroad. Where the assessment was not completed by us, we will ask to review the diagnostic report before we can offer an appointment. 
 
The appointments can be held face-to-face in our Redhill clinic, or via videoconference. The appointment will last approximately 60 minutes. 
 
The process for starting ADHD medication requires a number of important steps to be undertaken prior to initiating treatment. ADHD medication should be initiated by a healthcare professional with expertise in the management of ADHD. 
We undertake an individualised approach to starting treatment which relies on a deep understanding of the person's difficulties which related to ADHD as well as their motivation for wanting to start treatment. We undertake the following steps when developing an individualised treatment plan with you: 
 
we will discuss the different treatment options available for treatment of ADHD and you will be provided with written information about these treatments 
we will agree on the outcome that you want by taking medication for ADHD and any concerns that you might have about starting treatment 
we will complete a cardiovascular history which is completed when you register with us 
we will check your height, weight, blood pressure and pulse rate before starting medication. When attending a face-to-face appointment, we may also complete a cardiac auscultation (listening to the heart with a stethoscope) 
we will discuss the process for starting medication privately and issue a private prescription which will be sent to one of our preferred community pharmacies. More information about this can be found in our Frequently Asked Questions  
we will also discuss any further referrals or treatment that you might benefit from that cannot be provided by ADHD Treatments Surrey 
we will write to your GP to let them know of the outcome of your consultation within 2 week 
after starting medication, we will need to be in touch for ongoing care (add link to section below) 

Ongoing care 

Medication for ADHD is considered specialist only in the UK. This means that it can usually only be started and titrated by a specialist with training in the assessment and treatment of ADHD. Therefore, once medication has been started, you will need to be seen regularly by a specialist at least until your medication is stable and titrated to an optimum dose. This takes different time for different people, with an average of 3-4 months. 
 
The appointments can be held face-to-face in our Redhill clinic, or via videoconference. The appointment will last up to 45 minutes. 

Follow-up appointments

Your appointment will last for up to 45 minutes. 

At this appointment: 

we will discuss the impact that medication has made on your ADHD symptoms 
We will discuss any side effects that you are experiencing and manage them accordingly 
we will assess your mental state and ADHD symptoms 
we will decide whether you would like to have any further dose changes or if you are happy with the outcome of the treatment 
we will take weight, pulse rate, blood pressure and height (under 18s only) readings to ensure your medication remains safe for you to take 
if you are happy with the outcome of the treatment and we agree that no further drug or dose changes are required, we can make a request to your GP to take over the prescribing of your ADHD medication and complete regular physical health monitoring. This is known as a Shared Care Agreement*. Both you and your GP will need to agree to shared care for the GP to take over the prescribing and physical monitoring of your ADHD medication 
if your GP agrees to the Shared Care Agreement then you will need to be seen by an ADHD specialist every six to twelve months in order to continue treatment. You may decide to continue to be seen by ADHD Treatments Surrey for these specialist reviews or you may want to ask your GP to be referred to your local NHS ADHD service for further follow-up, depending on their waiting times 
if your GP rejects the request for Shared Care, you can continue to obtain your medication through the clinic. We can issue up to 3 months of medication at a time (split over 3 monthly prescriptions). We will continue to monitor your weight, blood pressure and pulse in the meantime and request to see you for a follow-up appointment every 6-12 months. 
 
Following the consultations, a brief letter will be sent to your GP within 2 weeks. 
 
*Shared Care is not something GPs are obliged to take on and requests for shared care may be turned down. You are advised to confirm your GP surgery is happy to accept Shared Care Requests from a private specialist independent prescriber before booking an appointment.
